We are looking for Adult Gerontology Nurse Practitioner to provide primary care services as part of our primary care team. The AGNP will work with a multidisciplinary team to care for and provide medical services to patients, including examining, conducting tests, analyzing records, reports, and test results, preventing and/or diagnosing illness, disease or injury, prescribing and administering treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury, explaining procedures and test results in the prescribed treatment plan, and managing labs and referrals.

The AGNP will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team. Perform other job-related duties as required by departmental needs or demands.

• Completion of an approved/accredited educational program for Nurse Practitioners
• Valid NY State license, CDS, DEA, and BLS
• Ability to effectively communicate, follow established procedures and protocols
• Understanding of and passion for community health care preferred
