• Overseeing clientele that is assigned to your care
• Creating and maintaining relationships with our caregivers and nurses caring for your client
• Executing, implementing, and updating the plan of care for the client, ensuring the caregiver is aware of changes
• Home visits – 75% of your time will be spent out in the field
• Onboarding prospective clients
• Monitoring discharge from hospital, SNF, or rehab
• Completing administrative paperwork in accordance with our policy and NYS DOH

• NYS registered nurse with up‑to‑date license
• Ability to work autonomously
• Ability to work some weekends throughout the month
• Must feel comfortable clinically training RNs and caregivers on‑site to their respective duties for their clients
