Now Hiring: RN – Home Health Nurse Assessor
Location: Brooklyn, NY (Choose Your Preferred Area)

- Midwood-Flatbush
Position: RN – Home Health Nurse Assessor
Duration: 12 Months Contract + Extension (Up to 5 Years)
Schedule: Day Shift | Hybrid (3 Days Onsite, 2 Days Remote)
Hours: Up to 40 Hours per Week
Training: 1 Month (1–2 Weeks Remote + 1–2 Weeks Shadowing)
Pay Range: $54 - $55/hr + $90/weekly for mileage
Requirements:
- Active RN License
- Minimum 2 Years of Home Health/Home Care Experience (within the last 15 years)
- Strong Clinical Assessment & Documentation Skills
- Excellent Communication and Patient Care Skills
What You'll Do:
- Evaluate patient needs and determine appropriate levels of care
- Complete accurate clinical documentation and assessments
- Collaborate with interdisciplinary healthcare teams
- Support quality patient outcomes through thorough evaluations
