Primary Care Nurse Practitioner / Physician Assistant
About the Opportunity
Our client, a growing community-based healthcare organization in Brooklyn, is seeking an experienced NP or PA to join its clinical team! This position will provide comprehensive primary care services to a diverse patient population while supporting preventive health and chronic disease management.
The annual base salary range is $145,000 to $180,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities- Conduct patient evaluations, medical histories, and physical examinations
- Diagnose and manage acute and chronic medical conditions, including diabetes and other long-term illnesses
- Develop individualized treatment plans and medication management strategies
- Order and interpret diagnostic tests and laboratory results
- Provide patient education on disease prevention, treatment plans, and lifestyle management
- Coordinate care by referring patients to specialists when appropriate
- Collaborate closely with physicians, nurses, and clinical staff to ensure high-quality patient care
- Document patient encounters accurately in the electronic medical record
- Follow up with patients to monitor progress and adjust treatment plans as needed
- Participate in clinical meetings and quality improvement initiatives
- Maintain patient confidentiality and comply with health care regulations
- Graduate of an accredited NP or PA
- Active New York State Medical license
- Minimum 3 years of clinical experience as a Provider
- Experience managing chronic diseases such as diabetes, hypertension and other long-term conditions
- Comfortable working with diverse and underserved patient populations
- Strong communication and patient education skills
- Must speak Spanish
- Full-time or part-time opportunities available
- Flexible schedule with occasional weekend coverage (approximately once per month)
