Compensation: $500 – $600 per day (1099)
A medical group providing primary care services to nursing homes is seeking a Nurse Practitioner to provide weekend coverage for a skilled nursing facility located in the Canarsie section of Brooklyn.
This role focuses on providing primary care to geriatric patients in a nursing home environment. The provider will manage patient visits, follow-ups, and coordinate care with facility staff to ensure continuity of care over the weekend.
ScheduleThe provider will work weekends (Saturday and Sunday). Providers must be available between 8:00 AM and 6:00 PM
, with flexibility in structuring their day based on patient volume and facility needs.
- $500 – $600 per day
- 1099 independent contractor
- Additional earning potential depending on patient volume
- Provide primary care services to geriatric nursing home residents
- Conduct routine and urgent patient visits
- Manage chronic conditions and acute medical issues
- Coordinate care with nursing staff and facility leadership
- Participate in CCM and RPM programs when applicable
- Document patient encounters in facility EMR systems (Sigma Care or PCC)
- Active New York Nurse Practitioner license
- Active Medicare and Medicaid provider ID Experience working with geriatric patients preferred
- Skilled Nursing Facility experience preferred
- RN background in a nursing home or long-term care setting is highly valued
