RN Registered Nurse - Home Health

Compass Healthcare Consulting and Placement is conducting a search for an experienced Registered Nurse, for a Home Health RN opportunity for a Certified Home Health Agency. Qualified candidates will have prior Nursing experience within Home Care, Hospital or other clinical settings. The position requires making patient visits within your desired area. Available visit areas include:
Manhattan, the Bronx, Brooklyn, Rockland or Sullivan County.

The Home Health RN is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. This would include, conducting home visits, completion of all required documentation, communication with PCP, completion of 485 and all interim orders, update medication profile, care coordination with other disciplines, review and completion of case communication notes, daily review of follow-up items and incomplete documentation items noted in HCHB, participation in case conferencing with respective supervisor.

This is a Full Time Direct Hire Position offering Benefits and Competitive Salary.

• RN with an active NYS RN License in good standing
• Minimum of one year of experience in medical surgical nursing, hospital based nursing or Home Health care preferred. Will train RNs with Hospital experience!
• Experience working for a Hospital, Acute Care, CHHA, LHCSA or MLTC a PLUS!
• Will train RN with some working experience within a Clinical Setting
• Ability to visit patients in a home environment.
• Willingness to collaborate with Interdisciplinary Team as needed
• Excellent Organizational Skills Required.
• Driver's License & Access to reliable vehicle

Competitive Salary $105,000 - $134,000 based on experience and visit location. Some positions also have $15-20k sign-on bonus with Excellent Benefits including Generous PTO, Medical, Dental, Vision, Retirement Plan and more!
