Hospice Nurse Practitioner

Location:

Manhattan, NY (4 days on site, 1 day remote) — weekly hybrid work schedule.

Salary: $125,000 - $150,000 per year.

Sign‑on bonus: $15,000.

Provide direct skilled care to pediatric patients requiring palliative and hospice services in nursing homes, hospitals, and community settings. Oversee pain and symptom management, goal‑setting discussions, and hospice eligibility reviews.

• Conduct patient assessments (cognitive and physical). Follow up with multiple visits as needed.
• Take patient histories, perform examinations, and develop individualized care plans.
• Order diagnostic testing as appropriate.
• Formulate problem lists and recommend interventions aligned with a palliative care plan.
• Lead key elements of palliative care, including family meetings, goal‑setting discussions, and symptom management.
• Perform hospice eligibility reviews and engage in accurate hospice discussions.
• Collaborate with physicians and other team members.
• Evaluate patient response to treatments and modify plans when needed.
• Educate patients and families about disease processes and the end‑of‑life journey.
• Offer hospice choices, assist with handoffs to hospice providers when elected.

• Minimum of 2 years nursing experience in hospice or palliative care.
• Background in oncology, hospice, or palliative care with pediatrics emphasized.
• Ability to work independently in a patient‑centered environment.
