• Complete UAS NY assessments during in-home visits or via telehealth.
• Review medical documentation, record accurate detailed findings.
• Educate consumers and communicate with providers when needed.
• Maintain timely high quality documentation that meets all NYS standards.
• Manage your schedule, route planning, and daily workflow.
• Follow all conflict-free requirements under the NYS Department of Health.

What You Need:
Must have own car for daily travel between assessment locations. Public transportation and ride sharing are not permitted.

• 1–2 years of experience as an RN in one-on-one in-home, community, skilled nursing facility, long term and geriatric care.
• RN license valid in New York.
• BSN is required.
• Familiarity with Uniform Assessment System (UAS)-NY is a plus.
• Strong communication, time management, and Electronic Health Record (EHR) skills.
• Reliable transportation for in-person visits.

• Active RN license.
• At least 22 months of home health care experience within the last 15 years, including one or more of the following:
  • RN/LPN experience with a CHHA, LHCSA, or MLTCP.
  • RN Care Manager for home- or community-based services (Hospice/Assisted Living included).
  • NP providing primary care to homebound individuals.
  • HHA or Personal Assistant experience in certified/licensed home care settings.

Must meet conflict-free requirements under NYS DOH.

• Recent experience completing OASIS UAS CHA or pediatric assessments.
• Experience managing chronic conditions in older adults.
• Combination of Home Health Aide experience and private duty RN or LPN work.
• Familiarity with the Uniform Assessment System for New York.

• RN Case Manager experience in hospitals or health plans.
• Experience with telephonic only assessments.
• RN work in group home or residential settings.
