Job Description
Experience: Mid Level
Salary: $115,000 - $120,000 per year
Homecare Coordinator of Care / CHHA RN will oversee the process of a patient’s recovery from illness, injury or surgery. RN will do 5 to 6 patient visits per day (Monday thru Friday 8:30 am to 5 pm). Territory:
Brooklyn, NY.
- Implements and evaluates the home care needs of patients, including the assessment of a safe living environment, proper resources and support system of caregivers.
- Communicates a plan of care for the patient between the physician, physical therapist, social worker and home health aide services.
- Reconciles patient's medications with the physician.
- Assesses patient and caregivers' knowledge of medications to instruct on medication use, actions, side effects, dosage, routes, and frequencies.
- Evaluates and implements appropriate interventions for patient’s wounds, signs and symptoms of disease processes and any complications.
- Educates patients and caregivers on treatment of disease and tools for disease management.
- Proficient in placing wound vacs and Unna Boots when instructed by the physician.
- Completes and submits OASIS assessments, reassessments, transfers, resumption of care, discharges, and significant changes in conditions in accordance with Agency defined time frames.
- Reviews patient insurance information to coordinate activities involved in each individual care plan.
- Assures that pertinent findings are shared with family and professional caregivers.
- Enters all relevant patient visit documentation into the Kan Time EMR.
- RN with active New York State Nursing license in good standing.
- Homecare / CHHA experience preferred.
- OASIS experience highly preferred.
- RN must have a car/vehicle.
