Salary: $126,500 - $148,500 (wider range available depending on experience and location)

Role Description

The role provides effective and equitable value‑based primary care to adults on Medicare, delivering care independently and coordinating with physicians and team members.

Core Responsibilities
  • Conduct office visits for routine and acute issues.
  • Administer annual wellness visits and health risk assessments.
  • Care coordination with other providers, specialists, testing facilities, and agencies.
  • Population health leadership, ensuring eligible patients receive evidence‑based screenings.
  • Assist care team with phone triage and outreach.
  • Educate patients on health conditions, care plans, and treatments.
  • Participate in Oak Street Health promotional activities.
  • Conduct home visits as needed.
  • Other duties as assigned.
Required Qualifications
  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification in at least one of:
    Family Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner.
  • Active, non‑probationary state Nurse Practitioner license.
  • Active DEA license.
  • U.S. work authorization.
Preferred Qualifications
  • Experience in primary care (internal medicine, geriatrics, or family medicine).
  • Understanding of HCC documentation, ICD‑10 coding, and Health Risk Assessments.
  • Strong teamwork and cross‑functional collaboration skills.
  • Passion for empirically‑guided population health.
  • Bilingual proficiency in applicable areas.

Anticipated weekly hours: 40

Pay Range: $78,331.00 - $ (base salary varies with experience, education, geography, and other factors)
