- Professional requirements include Driver's License/State , 2 Professional References, Criminal Background Check, Resume, Webchecks, National and State Sex Offender Registry checks, Education Verification, Client Forms (Security Credentials Acknowledgement Form and Confidentiality Agreement), Employment Verification
- Checklist/Testing requirements include Core Competencies, Unit Exam, Skills Checklist
- Bassett Drug Screen required
- Submittal Profile includes Resume, Skills Checklist, Nursys and/or License Verification, BLS, and any additional required certifications
