Role DescriptionOur purpose is to provide effective and equitable value‑based primary care to adults on Medicare, keeping them healthy and out of the hospital. Nurse Practitioners see patients independently, collaborate with physicians, and provide longitudinal care and meaningful patient relationships to build trust and connection.
Core Responsibilities- Provision of exceptional primary care
- Conduct office visits for routine and acute issues
- Administer annual wellness visits and health risk assessments with a holistic view and thoughtful documentation
- Coordinate care with other providers, specialists, testing facilities, and agencies
- Lead population health initiatives, such as ensuring eligible females receive breast‑cancer screening every two years
- Assist the care team with phone triage and outreach
- Educate patients on health conditions, care plans, and treatments
- Participate in Oak Street Health promotional activities
- Conduct home visits as needed
- Perform other duties as assigned
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in Family Nurse Practitioner, Adult‑Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active, non‑probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
- Bilingual proficiency in Spanish
- Experience in primary care: internal medicine, geriatrics, or family medicine
- Understanding of HCC documentation, ICD‑10 coding, and Health Risk Assessments (HRAs)
- Passion for teamwork and collaboration across functions
- Desire to contribute to an innovative model focused on empirically‑guided population health
