As a team member on our Senior Community Care (SCC) product, you will provide care to patients at home, nursing homes, assisted living, and senior housing. This life‑changing work adds a layer of support to improve access to care.

The hours are flexible; weekday availability is preferred and shifts can be 8‑4, 9‑5 or 10‑6.
Primary Responsibilities- Primary Care Delivery
- Deliver cost‑effective, quality care to assigned members
- Manage both medical and behavioral, chronic and acute conditions effectively, in collaboration with a physician or specialty provider
- Perform comprehensive assessments and document findings in a concise, compliant manner that meets CMS regulations
- Ensure all diagnoses are ICD‑10 coded accurately and documented appropriately
- Address all quality elements as required by the APC
- Conduct initial medication review, annual medication review and post‑hospitalization medication reconciliation
- Facilitate agreement and implementation of the member’s plan of care by engaging facility staff, families, and physicians
- Evaluate the effectiveness, necessity and efficiency of the plan, making revisions as needed
- Utilize practice guidelines and protocols established by CCM
- Complete all mandatory educational and Learn Source training requirements
- Travel between care sites (mandatory)
- Care Coordination
- Understand payer/plan benefits, CCM associate policies, and procedures, and articulate them to providers, members and stakeholders
- Assess medical necessity and effectiveness of ancillary services to determine appropriate benefit events, communicating the process to providers
- Coordinate care as members transition through different levels and settings
- Monitor needs of members and families, facilitating adjustments to the plan of care
- Review orders and interventions for appropriateness, ensuring the most effective plan of care aligns with member needs
- Evaluate plan cost‑effectiveness while meeting member, family, and provider needs to reduce high costs and unnecessary hospitalizations
- Program Enhancement Expected Behaviors
- Maintain regular and effective communication with internal and external parties, including physicians, members, key decision makers, nursing facilities and CCM staff
- Actively promote the CCM program in assigned facilities, partnering with stakeholders to grow membership
- Demonstrate original thinking and creativity in developing improved methods
- Work independently with minimal supervision
- Schedule available hours in web‑based application at least one month in advance
- Show initiative in achieving goals and objectives
- Participate in CCM quality initiatives
- Check Optum email intermittently for required training, communications and monthly scheduling
- Certified Nurse Practitioner or Physician Assistant through a national board
- For NPs:
Graduate of an accredited master’s degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), Adult‑Gerontology Acute Care NP or other relevant certifications, with preferred certification as ANP, FNP, or GNP - Active and unrestricted license in the state of residence
- Current active DEA licensure or prescriptive authority, or ability to obtain post‑hire according to state regulations
- Reliable transportation to travel to client and/or patient sites within designated area
- Ability to lift a 30‑pound bag in and out of a car and navigate stairs and various dwelling conditions
- Ability to obtain a collaborative practice agreement, if required in the state
- 1+ years of hands‑on postgraduate experience within long‑term care
- Understanding of geriatrics and chronic illness
- Understanding of advanced illness and end‑of‑life discussions
- Proficient computer skills for documenting in electronic medical records
- Ability to develop and maintain positive customer relationships
- Adaptability to change
