Location: New York
Oak Street Health is seeking a Nurse Practitioner to deliver value-based primary care to adults on Medicare in outpatient clinics. You will work on a collaborative care team, conducting routine and acute visits, wellness checks, and health-risk assessments while building lasting patient relationships.
You will coordinate care with physicians, PAs, nurses, and care managers, lead population-health initiatives, and may perform home visits.
