Role DescriptionThe purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value‑based primary care to adults on Medicare, keeping them happy, healthy, and out of the hospital. Nurse Practitioners see patients independently and collaborate with physicians, depending on the patient’s specific situation. They provide longitudinal care and build meaningful patient relationships, fostering trust and helping patients understand their care plan.
Nurse Practitioners practice in outpatient settings on a collaborative care team composed of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. They partner with a network of elite specialists and hospitals for specialty and acute care, allowing providers to focus on primary care, care coordination, and population health within the clinic.
CoreResponsibilities
- Provision of exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer annual wellness visits and health risk assessments, focusing on holistic health and accurate documentation.
- Coordinate care with other providers, specialists, testing facilities, and agencies.
- Lead population health initiatives, ensuring eligible patients receive evidence‑based screenings.
- Assist the care team with phone triage and outreach.
- Educate patients on health conditions, care plans, and treatments.
- Participate in Oak Street Health promotional activities.
- Conduct home visits as needed.
- Perform other duties as assigned.
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- Active, non‑probationary state Nurse Practitioner license.
- Active DEA license.
- US Work Authorization.
National certification in at least one of the following specialties:
- Family Nurse Practitioner
- Adult‑Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Experience in primary care: internal medicine, geriatrics, or family medicine.
- Understands HCC (Hierarchical Condition Categories) documentation, ICD‑10 coding, and Health Risk Assessments.
- Passion for teamwork and cross‑functional collaboration.
- Desire to be part of an innovative, empirically‑guided population health model.
- Bilingual proficiency in Spanish.
