Position SummaryWe have an exciting opportunity to join our team as a APP Manager - PA or Certified Midwife. In this role, the successful candidate will provision, coordinate, and assure comprehensive health care using advanced practice processes and pertinent standards of care, diagnose illness and physical conditions, and perform diagnostic, therapeutic, corrective and prescriptive measures as appropriate for patients within a collaborative practice environment.
The manager will supervise and direct a designated group of Nurse Practitioners, Midwives and/or Physician Assistants for the purpose of optimizing service line activities in the delivery of health care to patients while promoting a positive work environment. They will serve as liaison to nursing, care management, hospital administration, and other hospital departments and represent the Office of Advanced Practice Providers to accrediting bodies ensuring institutional compliance.
- Perform all relevant clinical functions based on evidence‑based approach to clinical care, using established guidelines. Perform complete history, physical examination and psychosocial assessment within the designated practice.
- Work collaboratively with the practice team, attending physicians, house staff, consulting physician(s) and other disciplines to identify, develop, implement, create and modify as necessary an appropriate plan of care that considers potential outcomes and goals of care, maximizes patient/family satisfaction and enhances quality, access and cost outcomes.
- Order laboratory, radiological and diagnostic studies as indicated, differentiate between normal and abnormal findings and follow protocols for abnormal findings, which may include adjustment in plan of care or discussion with attendings and house staff.
- Prescribe medications to patients according to NYS law, including controlled substances with DEA authorization. Prescribe medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Evaluate patient responses to treatment and modify per clinical standards and practice protocols.
- Coordinate discharge planning with interdisciplinary team, patient and family to assure timely and safe discharge. Practice as a member of the allied health staff according to the rules and regulations, and bylaws of the Medical Staff. Practice within scope of practice and approved delineation of privileges.
- Act as a professional role model to all levels of staff by incorporating expertise and related experience into patient care, considering needs and behaviors of specific patient age and cultural groups in all patient care activities.
- Evaluate advanced practice activities that contribute to achievement of patient outcomes and add value to clinical care. Utilize practice‑based evidence as mechanism to evaluate care and the need for change in structure or processes to improve care.
