Location: New York
MJHS in Bronx, NY seeks a Hospice Field Admission Nurse (RN) to perform initial physical and psychosocial assessments, developing individualized plans of care for patients across home, hospital, or facility settings.
You will admit eligible patients to the Hospice program following NHPCO standards, collaborating with families and care teams to ensure high-quality, patient-centered care. NYS RN license and driver’s license required; 2+ years in med/surg, community health, or oncology preferred.
