Ambulatory Care Nurse – responsible for coordinating, integrating, communicating, implementing and evaluating nursing practice and clinical policies and procedures. The role includes quality and performance improvement activities, data collection, collaboration with licensed providers, and comprehensive patient care.

• Perform physical examinations and diagnostic tests.
• Collect patient health history.
• Record information in the electronic health record.
• Provide information, counseling, and education to patients and families regarding test results and treatment plans.
• Administer medications (including vaccination, IV therapy, oxygen administration), provide comprehensive wound care, and perform triage for clinical concerns.
• Assess and monitor patient responses to treatments, procedures, and anesthesia.
• Interpret patient information to make treatment decisions.
• Consult with clinical supervisors and licensed providers to determine best treatment plans.
• Direct, supervise, and delegate to licensed practical nurses, medical assistants, and technicians to support patient flow and efficiency.
• Support coordination, integration, communication, implementation, and performance evaluation of nursing practice and clinical policies and procedures.
• Ensure compliance with Patient Safety initiatives and reporting.
• Participate in quality initiatives such as MIPS and care‑gap management.
• Apply NYULH Service Standards to all responsibilities.
• Participate in activities related to NYULH clinical standards and external regulations.
• Collaborate with interdisciplinary team members and promote collegial relationships.
• Serve as resource for nursing and other clinical staff.
• Participate in clinical inquiry and evidence‑based practice activities.
• Perform emergency care in accordance with organizational policy.
• Delegate and oversee specific nursing interventions to team members as appropriate.
• Execute medical regimens prescribed by licensed providers.
• Assess patients, identify unmet needs, and plan comprehensive care.
• Identify and report co‑morbidities, complications, or emergent conditions.
• Integrate care with other healthcare providers such as respiratory therapists, physical therapists, social workers, and dieticians.
• Provide direct care, including administration of treatments and medications, disease prevention guidance, rehabilitation, public health care, and symptom relief.
• Document clinical history and assessment data.
• Perform onsite testing (POC lab testing, EKG, specimen collection, etc.).
• Perform medical and nursing treatments per plan of care and provider orders.
• Perform procedures including hot & cold applications, orthopedic appliance management, suturing, catheter care, and wound care.
• Assist with minor and major procedures.
• Monitor patient status pre‑, intra‑, and post‑procedure.
• Administer medications via all routes and facilitate medication refills.
• Manage IV therapy including blood products and chemotherapy.
• Maintain peripheral and central IV lines, addressing complications with proper certification.
• Manage clinical in‑baskets and close encounters promptly.
• Document and report critical lab values.
• Relay results, follow‑up, and explanations to patients and providers.
• Triangulate patient concerns and provide clinical advice within RN scope.
• Perform case management of a defined population via multidisciplinary coordination.
• Identify patient, family, and community health deviations and advocate for appropriate actions.
• Analyze assessment data to determine nursing diagnoses and expected outcomes.
• Escalate urgent or emergent deviations to medical providers or emergency care.
• Promote individualized care, considering patient age, cultural group, and behavioral needs.
