Per Diem LPN - Mattituck

We have an exciting opportunity to join our team as a Per Diem LPN in Mattituck. The successful candidate will use clinical experience to plan, implement, and evaluate safe, therapeutic, age‑appropriate patient care. The LPN works collaboratively within a multidisciplinary health team to promote wellness and optimize patient outcomes while complying with all hospital policies, federal, state, and local regulations, and HIPAA guidelines.

• Perform other duties as assigned.
• Complete documentation reflecting multidisciplinary coordination and patient/family teaching, including discharge preparation.
• Evaluate patient response to nursing and multidisciplinary interventions, modify care plans as needed, and collaborate with the RN to optimize outcomes.
• Identify and resolve clinical issues at point of care and participate in risk management, quality care, and ethical issue investigations.
• Maintain clear communication with patients, families, and the health team; document advance care directives and verify family‑centered care principles.
• Manage medication orders, prior authorizations, and referrals through the Workflow system, ensuring correct dosing and compliance with physician approvals.
• Ensure a clean, safe therapeutic environment, assist with emergency situations, and follow HEICS plans as required.
• Assist with patient satisfaction matters, implement conflict resolution techniques, and document patient/family rights as needed.

• High School or Equivalent Technical Diploma.
• Graduated from an accredited school of licensed practical nursing.
• 0‑1 year of nursing experience in a hospital, ambulatory, private practice, or home health care setting (preferred).
