Hourly Rate: $62.63 (subject to change based on shift differential, experience, education or other relevant factors)
- Leadership in patient care coordination:
- Coordinates an interdisciplinary plan of care.
- Assures 100% of assigned patients have a documented plan of care.
- Ensures completion of patient‑care assignments 100% of the time.
- Ensures the plan of care is congruent with accepted LOS parameters for all assigned patients.
- Delegates aspects of care to team members consistent with their capabilities and responsibilities.
- Ensures delegated care is completed in accordance with established standards of practice.
- Negotiates with team members if unable to fulfill role responsibilities; escalates as per MMC policy and procedure.
- Establishes a therapeutic relationship with patient/family.
- Assumes charge nurse assignment.
- Mentors and precepts associates during orientation and development.
- Documents patient/family wishes related to care decisions.
- Systematic assessment of patient health and care response:
- Completes 100% of admission assessments within 8 hours.
- Documents interdisciplinary team approach per policy.
- Collects data systematically and continuously.
- Documents teaching/learning needs of patients and families prior to discharge.
- Assesses and documents patient response to nursing and interdisciplinary interventions.
- Initiates teaching and discharge plan for 100% of assigned patients within 24 hours of admission.
- Modifies plan of care through prioritization of patient/family needs and interdisciplinary orders.
- Critical thinking and clinical decision-making:
- Takes action to optimize achievement of patient outcomes.
- Participates in identification, investigation, and resolution of risk management, quality care, and ethical issues.
- Incorporates problem-solving, conflict resolution, and consensus building into daily practice.
- Patient progress evaluation and plan adjustment:
- Identifies variances to plan of care.
- Initiates appropriate interventions to amend identified variances.
- Documents patient and family response to interventions and modifies plan accordingly.
- Reviews plan of care daily and updates to reflect current status.
- Consistently participates and communicates in daily IDT rounds to provide concise direction to all caregivers.
- Direct patient care development and coordination:
- Provides patient‑focused care.
- Incorporates standards of practice, critical pathways, age‑specific guidelines, and patient care guidelines into the plan of care.
- Assesses potential and actual changes in level of care required.
- Develops plan of care within 24 hours of admission when possible.
- Involves patients/families in care planning.
- Safe environment provision:
- Orients patient/family to the environment, ensuring comprehension of presented information.
- Administers medication per medication administration policy.
- Responds appropriately to urgent/emergent situations.
- Documents use of restraints according to hospital patient care guidelines.
- Uses equipment in accordance with MMC policies and procedures.
- Comprehensive direct patient care:
- Utilizes nursing knowledge, research, and evidence‑based practice in application of care and clinical judgment.
- Administers care consistent with standards of care for MM.
- Considers patient’s biopsychosocial and spiritual needs in interventions.
- Effective communication:
- Defends and preserves patient rights in all aspects of care, information, knowledge, and privacy.
- Demonstrates respect for cultures and cultural diversity among co‑workers and the healthcare team.
