We are seeking a compassionate and skilled Licensed Practical Nurse LPN to work in our skilled nursing facility. As an LPN at Pine Haven Nursing & Rehabilitation Center, you will be responsible for:

• Providing direct patient care to our residents, including administering medications, performing vital signs, and monitoring their health status

• Collaborating with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of our residents

• Maintaining accurate and up‑to‑date medical records and reporting any changes in a resident's condition to the healthcare team

• Provide high‑quality patient care in accordance with established nursing standards and protocols

• Conduct initial and ongoing assessments of residents' physical and emotional needs

• Develop and implement individualized care plans with input from the healthcare team

• Administer medications and treatments as prescribed by physicians
