OverviewLicensed Practical Nurse (LPN) — Full-Time or Part-Time opportunities. Rate: $31 - $34.24/hour (depending on experience). Sign-on Bonus:
Up to $3,000

Schedule:
Days (6:30 am – 7:30 pm, every other weekend) and Nights (6:30 pm – 7:30 am, every other weekend).

Responsibilities- Provide nursing care in accordance with resident assessments, care plans, and facility policies
- Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice
- Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties
- Accurately document care, treatments, observations, and medication administration
- Supervise and support Nursing Assistants to ensure quality resident care
- Maintain up-to-date resident care information and communicate pertinent information during shift changes
- Follow infection control, safety, resident rights, and confidentiality standards
- Participate in interdisciplinary care planning and quality improvement initiatives
- Assist with admissions, transfers, and other nursing responsibilities as assigned
- High School Diploma or equivalent required;
Associate's Degree preferred - Valid NYS Licensed Practical Nurse (LPN) License required
- Current CPR Certification required
- Experience in a skilled nursing facility or with the geriatric population preferred
- Strong clinical, communication, and organizational skills
- Knowledge of medication administration, infection control, and resident care practices
- Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
- Compassionate, resident-centered approach to care
- Education:
High School Diploma or equivalent;
Associate’s Degree preferred - Licenses &
Certifications:
NYS LPN license; CPR certification - Experience:
Experience in skilled nursing or geriatric population preferred
