• Provide direct patient care to residents, including administering medications, wound care, and IV therapy as needed
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Conduct regular assessments of patients' physical and emotional needs, and document all care provided
• Collaborate with other healthcare professionals to ensure seamless communication and coordination of care
• Participate in maintaining a safe and clean environment for residents and staff
• Demonstrate a commitment to continuous learning and professional development

• Current licensure as a Registered Nurse in the State of New York
• Prior experience in a long-term care setting preferred (
