Infection Control and Prevention Nurse RN

Overview

Infection Prevention & Control Nurse (RN) at Fairview Skilled Rehab & Nursing Home is a role to lead and oversee the facility's infection prevention and control program. This position is responsible for monitoring infections, ensuring regulatory compliance, educating staff, conducting surveillance, and implementing evidence-based practices to protect residents, visitors, and staff.

• Oversee the Infection Prevention & Control Program in accordance with CDC, CMS, and NYS DOH guidelines.
• Conduct infection surveillance, tracking, reporting, and trend analysis.
• Investigate and manage outbreaks while implementing containment measures.
• Educate and train staff on infection prevention practices, PPE, hand hygiene, and isolation precautions.
• Perform infection control rounds and audits throughout the facility.
• Monitor antibiotic stewardship initiatives.
• Ensure compliance during state surveys and regulatory inspections.
• Collaborate with medical providers, nursing leadership, and interdisciplinary teams to improve resident outcomes.
• Maintain accurate documentation and required infection control reports.

• Current New York State Registered Nurse (RN) license.
• Infection Prevention experience in a skilled nursing, rehabilitation, or long-term care setting preferred.
• Certification in Infection Control (CIC) preferred or willingness to obtain.
• Strong knowledge of CDC, CMS, OSHA, and NYS Department of Health regulations.
• Excellent communication, leadership, organizational, and problem-solving skills.
