Nurse Practitioner (NP) / Physician Assistant (PA) — Nursing Home Primary Care | Brooklyn, NY (Canarsie)

Location: Brooklyn, NY (Canarsie)

Compensation: $500 – $600 per day (1099)

Schedule: Full-Time | Monday–Friday | Availability between 8:00 AM – 6:00 PM

A medical group providing primary care services to nursing homes is seeking a Nurse Practitioner or Physician Assistant to provide ongoing coverage for a skilled nursing facility located in the Canarsie section of Brooklyn.

This role focuses on providing primary care to geriatric patients in a nursing home environment. The provider will manage patient visits, follow‑ups, and coordinate care with facility staff to ensure continuity of care.

The provider will be responsible for managing a panel of nursing home patients, addressing both routine and acute medical needs while ensuring high‑quality, coordinated care in collaboration with facility staff.

• Provide primary care services to geriatric nursing home residents
• Conduct routine and urgent patient visits
• Manage chronic conditions and acute medical issues
• Coordinate care with nursing staff and facility leadership
• Participate in CCM and RPM programs when applicable
• Document patient encounters in facility EMR systems (Sigma Care or PCC)

• Active New York NP or PA license
• Active Medicare and Medicaid provider
• Experience working with geriatric patients preferred
• Skilled Nursing Facility experience preferred
• RN background in a nursing home or long‑term care setting is highly valued
