TAG Med Staffing is hiring a Community Registered Nurse (RN) in Brooklyn, NY for our client. In this temp-to-perm, field-based role, you will provide community nursing, home visits, care coordination, discharge planning, medication management, and clinical support for individuals with intellectual and developmental disabilities (IDD) and behavioral health needs. The Community Registered Nurse (RN) will work independently while partnering with interdisciplinary teams to support safe transitions and continuity of care.
Responsibilities- Community Assessments: Conduct nursing assessments for prospective admissions, complete home visits, and evaluate individuals transitioning from hospitals into community-based programs.
- Clinical Care: Provide skilled nursing services including medication management, chronic disease monitoring, wound care, health assessments, and other nursing interventions based on individual needs.
- Care Coordination: Coordinate hospital discharges, collaborate with healthcare providers, and facilitate continuity of medical, behavioral health, and community-based services.
- Medication Oversight: Review medication administration records, monitor therapeutic outcomes, oversee medication storage, and maintain compliance with nursing and regulatory standards.
- Collaboration: Partner with physicians, case managers, behavioral health professionals, families, and direct support staff to develop and implement individualized care plans while providing clinical education and support.
- Education: Graduate of an accredited nursing program.
- Licensure: Active and unrestricted New York State Registered Nurse (RN) license is required.
- Experience: Minimum of 2 years of nursing experience with community health, behavioral health, care coordination, crisis intervention, or intellectual and developmental disabilities (IDD).
- Industry Knowledge: Familiarity with OPWDD, OMH, DOH, and community-based nursing practices is preferred.
- Technical
Skills:
Proficiency with electronic medical records (EMR), clinical documentation, nursing assessments, medication management, and care coordination is required. - Skills & Attributes: Strong clinical judgment, organization, communication, time management, and the ability to work independently while collaborating effectively within interdisciplinary teams are essential.
