Administer skilled nursing care for clients of all ages in their place of residence. Coordinate care with the interdisciplinary team, patients, families, and referring agencies. Assess home health and hospice patients to identify physical, psychosocial, and environmental needs. Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. Educate patients and families regarding care, medications, and home safety.
Supervise and provide clinical direction to home health aides to ensure quality service. Monitor assigned cases for compliance with third-party payer requirements.
Critical Information On-call responsibilities for patients and families as assigned. Home care environment. May need on-call coverage occasionally.
Education/Licenses Needed- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
- Current state license as a Registered Nurse.
- CHPN certification preferred.
- Proof of current CPR certification.
