About This Role

The Registered Nurse, Care Manager will coordinate, negotiate, procure, and manage patient care, providing focused care coordination across the acute care continuum. This role is ideal for a nurse with strong clinical expertise and excellent interpersonal skills.

• Schedule: Full-Time, Day shift

• Coordinate care of patients by providing focused care coordination across the acute care continuum.
• Evaluate appropriate clinical resource utilization and assess patients for transitioning to the next level of care.
• Collaborate with the health care team to ensure quality outcomes for patients and families.
• Identify cases requiring peer review and participate in quality/process improvement initiatives.
• Utilize protocols and pathways to formulate and implement patient plans of care.
• Communicate outcomes of chart reviews and managed care company reviews with the health care team.
• Advocate for patients in cases with insurance-related issues and delays in treatments.
• Assess patient and medical record documentation for appropriate admission and level of care.
• Document assessments, findings, progress, and recommendations in care management software.
• Participate in development of quality indicators and analysis for performance improvement.

• Education:
  
  BSN required, or graduate of an accredited RN program with a BS in a related health care field.
• Experience:
  
  3-5 years clinical experience, acute medical-surgical preferred.
• Licenses &
  
  Certifications:
  
  Current Registered Nurse License in New York State.

  Basic Life Support Certification required.
