About This Role

The RN Transplant Coordinator is a Registered Professional Nurse responsible for coordinating clinical programs and services for designated patient populations within the Transplantation Institute. This role is ideal for experienced nurses with a passion for transplant services and patient care.

• Pay: $ - $153,000 Annually
• Location: New York, NY
• Facility: Mount Sinai
• Department/Unit: Pre Renal Transplant - MSH
• Schedule: Full-Time

• Coordinate multidisciplinary screening and evaluation of transplant patients.
• Ensure all necessary lab tests are performed on potential transplant candidates.
• Participate in recommendations for placing patients on the transplant waiting list.
• Communicate with active transplant candidates regarding waiting list status.
• Facilitate organ offers from UNOS and communicate with surgeons and patients.
• Follow patients during hospitalization and coordinate outpatient follow-up visits.
• Educate patients and families on transplant medications and lifestyle changes.
• Assess patient recovery through compliance with outpatient visits and lab reviews.
• Collaborate with healthcare team to develop and review policies and standards of care.
• Participate in continuous quality improvement programs and risk management activities.

• Education:
  • Baccalaureate Degree in Nursing required
  • Masters in Nursing and certification in specialty area preferred
• Experience:
  • Progressive job-related experience specific to area of responsibility
