LVN ​/ LPN – LTC​/SNF

Licensed Practical Nurse (LPN)

Seeking a licensed practical nurse (LPN) to provide compassionate patient care in a long-term acute care (LTAC) facility. The ideal candidate will demonstrate strong clinical skills and flexibility to meet patient and unit needs.

Responsibilities:

• Administer medications and treatments in accordance with physician orders and facility protocols.
• Cover CNA duties as needed to support team functions.
• Monitor patient health status and document findings accurately.
• Ensure compliance with all licensing, safety, and quality standards.
• Assist with patient assessments and care plans.
• Complete all required documentation promptly and accurately.

Qualifications:

• Valid LPN license with active certification on file at submission.
• Minimum of 1 year of LTC experience.
• Current AHA BLS certification (attach proof).
• Ability to pass a 7-year background check, FACIS 3, and NY State check.
• Reliable transportation required; personal or rental vehicle strongly recommended.
• Proven ability to work flexible schedules, including every other weekend.
