Senior Licensed Practical Nurse

OPWDD is the safety net provider to individuals with Intellectual and Developmental Disabilities. As such, we serve individuals with the highest level of medical and behavioral support needs. Nursing is critical to the safe care and oversight of these individuals. With an aging population, the individual's medical/nursing needs have increased and will continue to increase. The senior licensed practical Nurse provides nursing care within their scope under the oversight of the Registered Nurse.

This Sr. LPN position will provide nursing services to the individuals within the Sinpatch team with varying medical needs. The Sr. LPN may be asked to provide coverage at other sites within the team if care is needed.

Health and Safety
- The Sr. licensed practical Nurse's role is essential to the health and safety of the individuals we serve. The Sr. LPN schedules and attends appointments for increased medical acuity individuals, completes medication audits, provides treatments to the individuals and documents the provision of services, as reflected in their life plan, safeguard summary plan, etc.

Essentially, the goal of the Sr. LPN is to help people with developmental disabilities live richer lives. To accomplish this, the Sr.

LPN will work towards increasing knowledge and delivering the highest quality supports and services for Taconic's Individuals.

Revenue Generation
- The Sr. LPN is an integral member of the care team. LPNs provide documentary evidence of individual's well being.

The Senior Licensed Practical Nurse 2 (SLPN2) will be assigned to the Sinpatch Team, which encompasses the Mistler, Spruce, Heritage, and Riverview IRA facilities. Mistler serves five individuals with varying medical needs, while Spruce accommodates four individuals, Heritage serves three, and Riverview also serves four, each with distinct medical and behavioral requirements. The SLPN2 will be based out of the Mistler location.

The SLPN2 performs the tasks and activities of a Senior Licensed Practical Nurse 1 listed below; and may supervise and train Senior Licensed Practical Nurses 1.

Primary duties and responsibilities:

  • Supervising the LPNs within the Sinpatch team
  • Developing patient care protocols and training staff on essential patient care procedures
  • Ensuring the implementation of medical plans and LPN nursing policies accurately, as well as timely recording of required data.
  • Effective communication between the IRA's residential staff and the Registered Nurse will be a priority
  • The Sr. LPN will actively participate in the treatment team to develop individual treatment plans
  • Ensure adherence to patient care standards and the timely delivery of services in accordance with these plans
  • Assess patient responses to treatments and medications and any changes in health status will be promptly reported to a Nurse II or physician.
  • Responsibilities also include monitoring and supervising insulin administration, administering influenza vaccines, and managing daily oral medications and treatments.
  • Accurate documentation, including The Ready to Go, monthly MARS, consents, and current prescriptions, must be maintained.
  • Address concerns raised by patients, families, and staff, escalating issues to higher-level personnel as necessary.
  • Maintaining comprehensive documentation of patient conditions and administered treatments is essential to this role.

You must have a valid driver's license to apply as travel is a part of the duties involved in this position. This may include a state-owned vehicle, as available, or the employee's own car. An employee who uses his/her own car will be reimbursed at current negotiated rates.

Promotion:

  • Possession of a license and current registration as a Licensed Practical Nurse in New York State; and one year of service as a Senior Licensed Practical Nurse 1.

Open Competitive:

  • A license and current registration to practice as a practical nurse in New York State AND two years of experience providing nursing care and treatment services to individuals under the supervision of a registered nurse; assisting with the implementation of treatment plans; administering, observing, and recording medication management; educating and counseling individuals and families regarding illnesses, medications, tests, and treatment goals and procedures;

    monitoring and recording individuals progress; and supervising direct care staff.

Transfer:

  • Candidates must have one year of permanent competitive, non-competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law.
