Medley Health is looking for a compassionate Physician or Nurse Practitioner for a part-time opportunity providing on-site physiatry and pain management services across assisted living facilities. You ll work independently with strong administrative support, collaborating with interdisciplinary teams to help residents regain function, improve mobility, and manage pain.
What You ll DoPerform comprehensive physiatry and rehabilitation evaluations and follow-up visits in assisted living settings
Diagnose and manage musculoskeletal, neurologic, and functional limitations in older adult populations
Develop individualized treatment plans focused on functional restoration, mobility, and pain management
Coordinate with PT, OT, and ST teams on plans of care
Document assessments and progress notes accurately and timely
Communicate recommendations to facility clinical teams and assist with care transitions as needed
Part-time schedule with a high level of autonomy
No nights, no weekends
Malpractice insurance coverage (as applicable)
Laptop provided
Strong administrative support and streamlined scheduling
Collaborative environment with nursing and therapy teams
- Physician (MD/DO)
MD or DO;
Board Certified or Board Eligible in PM&R preferred, or relevant experience in pain management, MSK, or rehab medicineActive New York medical license
Experience in assisted living, geriatric care, pain management, or related settings preferred
Strong clinical judgment, documentation, and communication skills
- Nurse Practitioner
NP license (Adult-Gerontology preferred; FNP considered with relevant experience)
Active New York NP license
Experience in assisted living, geriatrics, pain management, orthopedics, or internal medicine preferred
Ability to work autonomously and manage a patient panel
Strong documentation and communication skills
