We are seeking experienced Long Term Care Registered Nurses for a 13-week contract assignment in a skilled nursing/long-term care setting. This role focuses on providing consistent, high-quality nursing care to residents with chronic medical conditions and post-acute needs. The ideal candidate demonstrates strong clinical judgment, adaptability, and leadership in a structured care environment.
ResponsibilitiesProvide high-quality nursing care to residents in a long-term care/skilled nursing facility
Administer medications, perform treatments, wound care, and manage chronic conditions
Assess, plan, implement, and evaluate resident care plans in collaboration with the interdisciplinary healthcare team
Monitor resident conditions and promptly respond to changes in status
Supervise and support LPNs and CNAs as assigned
Serve as charge nurse when assigned
Maintain accurate documentation and comply with all facility, hospital, and nursing standards
Common diagnoses:
Dementia, Alzheimer's disease, diabetes, COPD, CHF, hypertension, post-acute recovery, and age-related conditions
ALC (Awaiting Long-Term Care Placement) residents
Typical patient ratios: 1:20-30 (may vary by facility)
RequirementsActive Registered Nurse (RN) license in New York State
Minimum of 1 successful traveler assignment completed (first-time travelers not considered)
BLS certification required (ACLS preferred)
Meditech experience required
Graduate of an accredited School of Nursing
Strong interpersonal, organizational, and communication skills
Ability to function effectively in a fast-paced long-term care environment
Demonstrated leadership, delegation, and prioritization skills
