Home Care Nurse (LPN or RN) – $35–$45/hr

We’re hiring Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide personalized, 1:1 care across Suffolk County.

What You’ll Do:

Deliver direct patient care and treatments based on individualized care plans

Administer medications and tailored interventions

Promote wellness through education, counseling, and preventive care Collaborate with multidisciplinary teams to coordinate care Maintain accurate and timely nursing documentation

Assist with mobility and physical support (must be able to lift up to 50 lbs)
Stay active and engaged during extended periods of standing or movement

Take on additional duties as needed to support patient well-being

What You’ll Need:

Active LPN or RN license issued by New York State Minimum 6 months of experience in a home care setting

Current BLS certification from AHAStrong communication and organizational skills

Empathy, reliability, and a commitment to high-quality care
