Cordial Health is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with psychiatric experience to provide on-site behavioral health care throughout Queens, New York
.
This full-time position offers a stable weekday schedule, meaningful clinical work and the opportunity to build lasting relationships with an underserved adult and geriatric population.
Responsibilities include:
- Conduct comprehensive psychiatric evaluations and diagnostic assessments
- Provide ongoing psychiatric consultations and follow-up care
- Diagnose and manage psychiatric and behavioral health conditions
- Develop individualized, evidence-based treatment plans
- Prescribe, monitor and optimize psychotropic medications
- Evaluate medication effectiveness, side effects and behavioral stability
- Collaborate with physicians, nursing staff, therapists, social workers and families
- Participate in interdisciplinary treatment planning
- Educate residents, caregivers and staff regarding diagnoses and interventions
- Provide compassionate, trauma-informed and resident-centered care
- Maintain accurate, timely and compliant clinical documentation
- Board-certified PMHNP-BC or Physician Assistant with significant psychiatric experience
- Active and unrestricted New York professional license
- Active DEA registration required
- Strong knowledge of psychiatric assessment, diagnosis, psychopharmacology and treatment planning
- Ability to manage a psychiatric caseload independently
- Excellent communication, organization and relationship-building skills
- Geriatric, skilled nursing, rehabilitation, long-term care or post-acute experience preferred
- Familiarity with electronic medical record systems preferred
