LVN ​/ LPN – LTC​/SNF

$1599.36/week –

Job Summary:

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting, focusing on patient well-being and safety. Administer treatments, document care, and collaborate with the healthcare team to ensure quality patient outcomes.

Responsibilities:

• Administer medications and treatments accurately following prescribed orders.
• Document patient care and interventions in the electronic health record system with EPIC experience.
• Monitor and report patient conditions, including behavioral and degenerative disorders.
• Float within skill set to different units as required, maintaining compliance with facility policies.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Adhere to all safety protocols and licensing requirements, including active NY LPN license.

Qualifications:

• Active New York LPN license; compact licenses not accepted.
• Minimum of 1 year of recent experience in a nursing facility or skilled nursing environment.
• BLS certification is required.
• Proficient in charting within EPIC and EKG/ECG procedures.
• Ability to float across units as needed and work evening shifts.
• Strong knowledge of behavior management and degenerative disorders.
