About the Opportunity

An established, multi-specialty outpatient healthcare organization in Brooklyn is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join its growing team as an in-person Telehealth Provider
.

This is an excellent opportunity for an NP or PA who enjoys providing high-quality primary and/or specialty care through telehealth while having the support and resources of an established medical facility. The provider will conduct virtual patient visits from the organization's Brooklyn location, working closely with an on-site clinical and administrative team.

The organization serves a diverse patient population and has an established patient base, allowing the provider to step into a structured environment with strong operational support rather than building a practice from the ground up.

Position Overview

The Nurse Practitioner or Physician Assistant will conduct scheduled telehealth visits with patients from a private clinical setting within the organization's Brooklyn facility. The provider will be responsible for evaluating patients, developing treatment plans, managing ongoing conditions, ordering appropriate diagnostic testing, prescribing medications when clinically appropriate, and coordinating care as needed within their respective scope of practice.

The position combines the flexibility and efficiency of telehealth with the benefits of working in a fully operational healthcare environment with on‑site support.

Key Responsibilities
  • Conduct comprehensive telehealth evaluations and follow‑up visits
  • Assess, diagnose, and treat patients within the provider's scope of practice
  • Develop individualized treatment plans based on patient needs and clinical findings
  • Manage acute and chronic medical conditions
  • Order and interpret appropriate laboratory and diagnostic studies
  • Prescribe medications and treatments in accordance with New York State regulations and applicable scope‑of‑practice requirements
  • Provide patient education regarding diagnoses, medications, preventive care, and treatment plans
  • Monitor patient progress and modify treatment plans when clinically indicated
  • Maintain accurate, timely, and compliant electronic medical records
  • Coordinate care with other providers and members of the clinical team
  • Make appropriate referrals to specialists and other healthcare services
  • Participate in care coordination and population health initiatives as appropriate
  • Maintain compliance with organizational policies, applicable regulations, and clinical standards
  • Provide a high level of professionalism, responsiveness, and patient‑centered care
Candidate Requirements
  • Active New York State Nurse Practitioner or Physician Assistant license
  • Current national board certification appropriate to the provider's profession and specialty
  • NCCPA certification for Physician Assistants or current national NP board certification for Nurse Practitioners
  • Active DEA registration, if required for the position
  • Active Medicare and Medicaid provider enrollment/identification
  • Strong clinical assessment and decision‑making skills
  • Excellent written and verbal communication abilities
  • Comfortable providing care through telehealth technology
  • Ability to independently evaluate patients and develop appropriate treatment plans within their scope of practice
  • Strong documentation and organizational skills
  • Ability to effectively manage a consistent patient volume
  • Commitment to quality, patient satisfaction, and evidence‑based care
  • Ability to work collaboratively with physicians, nurses, medical assistants, and administrative staff
Ideal Candidate

The ideal candidate is a confident, patient‑focused Nurse Practitioner or Physician Assistant who can work independently while also functioning effectively as part of a larger clinical team.

Candidates with experience in primary care, family medicine, internal medicine, outpatient medicine, community health, urgent care, or telehealth are encouraged to apply.

Experience working with a diverse or medically complex patient population is highly valued.

Schedule
