The Registered Nurse in a Skilled Nursing Facility provides comprehensive patient care through assessment, diagnosis, planning, implementation, and evaluation. This role is ideal for nurses who excel in collaboration and leadership within a healthcare team.

• Location: Manhasset, NY
• Facility: Northwell Health
• Schedule: Full-Time, Day shift

• Conduct patient interviews and assess health status across multiple systems.
• Communicate and document patient’s physiologic and psychological health status.
• Develop individualized patient outcome statements and goals.
• Coordinate cost-effective use of supplies and medications.
• Implement and maintain safety precautions in patient care.
• Provide emotional support and discharge instructions to patients and families.
• Measure effectiveness of care and perform variance analysis for improvement.
• Collaborate with care team members on treatment regimens.
• Administer medications safely and educate patients on drug interactions.
• Participate in patient and family education and discharge planning.

• Graduate from an accredited School of Nursing.
• Bachelor’s Degree in Nursing preferred.

• Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN degree within five (5) years of job entry date.

• Current License to practice as a Registered Professional Nurse in New York State required.
• Specialized certifications as needed.
