LPN – Long-Term Care
Schedule:
Evening shift, 8 hours. Assignment for 13-16 weeks. Bi-weekly orientations on Monday.
Assignment Details:
- Contract Length: 18 weeks.
Requirements:
- NY LPN license or proof of application with applied date.
- AHA BLS certification.
- Minimum of 1 year of LTC experience.
- Ability to cover CNA duties as needed.
Roles & Responsibilities:
- Provide nursing care to residents in a long-term care setting.
- Administer medications and treatments as prescribed.
- Assist residents with daily living activities.
- Monitor resident health and report any changes to the healthcare team.
- Collaborate with interdisciplinary teams to ensure high-quality care.
Compliance / Notes:
- Must work a minimum of every other weekend and adjust schedule as needed to meet unit demands.
- Required to work 2 out of 4 holidays (Thanksgiving, Christmas, Christmas Eve, New Year's Day).
- All employment gaps over 90 days must be explained.
- Reliable transportation is required as public transportation is not accessible in this area.
