Position OverviewReporting to the Administrator and Director of Nursing (DON) the Admissions Coordinator will screen prospective residents and coordinate their admission to the nursing facility in collaboration with the interdisciplinary team. The Coordinator also collects and distributes information to appropriate staff of all new residents, represents Schuyler Hospital and Seneca View Nursing accurately and honestly, acts with integrity and in the best interest of the hospital and nursing facility, and adheres to facility policy and regulations.
WhatYou’ll Do
- Screens, interviews, and evaluates all prospective residents and their representatives to determine suitability and eligibility of placement, including financial arrangements.
- Obtains and completes all necessary admission and readmission paperwork, including admission agreements, business agreements, insurance authorizations, PRI’s, and all other required documents prior to placement.
- Provides information to applications, their families and the referring providers related to admission to the facility.
- Maintains database tracking of all referrals and outcomes.
- Coordinates open and timely admission/referral communication of needs with nursing leadership daily; seeks nursing and provider approval for admission referrals where needed through the DON or Administrator.
- Schedules and provides tours of the facility where needed.
- Maintains a long‑term care bed waiting list.
- Completes resident new admission and discharge satisfaction surveys, reporting results to the facility QAPI Committee and appropriate feedback to department heads/leaders.
- Maintains open collaboration with referral sources and providers to ensure referrals and feedback are maintained.
- Graduated from a Licensed Practical Nurse, Social Work, or Registered Nurse program.
- Must have active NYS licensure.
- Other healthcare related degree.
- Two years of long‑term care experience, one year admission experience, or one year healthcare insurance experience.
- Repetitive use of hands and fingers (e.g., computer keyboard); may require lifting and carrying light loads (up to 40 lbs), including boxes, equipment, IV solutions and stooping and kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves and to file documents in lower file drawers); sitting, walking, or standing for long periods of time may be necessary;
must be able to physically operate the equipment used for the job.
