A P
ediatric Home Health Licensed Practical Nurse (LPN) in New York City and Long Island provides one-on-one, specialized medical care to infants, children, and adolescents with complex medical needs in their homes or school environments. Operating under the direct supervision of a Registered Nurse (RN) or physician, the LPN ensures the delivery of a customized physician-ordered Plan of Care while providing critical peace of mind to patients' families.

Clinical Responsibilities
  • Skilled Nursing Interventions
    :
    Provide 1:1 advanced care, including tracheostomy management, mechanical ventilator support, and gastrostomy tube (G-tube) feedings.
  • Medication Administration
    :
    Safely administer prescribed oral, topical, and aerosolized medications, as well as monitor patient responses.
  • Patient Assessment
    :
    Perform continuous monitoring of vital signs, physical symptoms, and behavioral changes, documenting findings accurately.
  • Emergency & Routine Care
    :
    Manage acute medical events, implement seizure protocols, perform wound care, and maintain oxygen delivery systems.
  • Escort & Mobility Support
    :
    Accompany pediatric patients safely to school, medical appointments, or community activities as mandated by their care plan.
Care Coordination & Communication
  • Clinical Reporting
    :
    Communicate critical changes in a patient's physical status directly to the supervising RN, clinical director, or primary care provider.
  • Family Education
    :
    Train parents and primary caregivers on daily symptom management, medical equipment safety, and emergency response protocols.
  • Documentation
    :
    Complete precise, real-time electronic documentation detailing treatments administered, fluid intake/output, and developmental milestones.
Job Requirements & Qualifications
  • Licensure
    :
    Active, clear license and current registration as a Licensed Practical Nurse (LPN) in New York State
    .
  • Certifications
    :
    Current Basic Life Support (BLS) / CPR certification from an accredited provider (in-person training preferred).
  • Experience
    :
    Minimum of 6 to 12 months of general nursing experience, with a preferred emphasis on pediatric care, high-tech home health, or intensive care.
  • Medical Clearances
    :
    Up-to-date physical examination (within the last 12 months), current PPD or QuantiFERON screen, and verified lab titers for MMR immunity.
Working Conditions & Scheduling
  • Shift Structures
    :
    Flexible shifts ranging across 8, 10, or 12 hours, with full-time arrangements available.
  • Locations
    :
    Travel is required to patient households throughout the five boroughs of New York City (Manhattan, Brooklyn, Queens, the Bronx, Staten Island) and across Long Island (Nassau and Suffolk counties).
