We are seeking experienced Registered Nurses (RNs) to join our Managed Long-Term Care (MLTC) program as UAS
Nurses
. This position is ideal for an RN with strong UAS assessment and field experience who is comfortable working independently in the community and using technology to complete clinical documentation.
The RN will conduct comprehensive in-person UAS reassessments
, evaluate changes in members' clinical and functional status, and ensure identified needs are accurately documented and communicated to the care team.
UAS experience is required. Candidates must also have prior field-based clinical experience and be comfortable using electronic systems and technology.
Key ResponsibilitiesConduct comprehensive in-person UAS reassessments of MLTC members.
Evaluate physical, cognitive, behavioral, functional, and social needs.
Assess Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
Review medications, diagnoses, current services, care needs, and available support systems.
Identify changes in condition that may require adjustments to the member's care plan or services.
Complete standardized assessments accurately and within required timelines.
Collaborate with care managers, providers, family members, and interdisciplinary team members.
Provide education and self-care guidance to members and caregivers as appropriate.
Maintain accurate and timely documentation using electronic systems.
Participate in quality assurance and assessment accuracy initiatives.
Follow applicable Medicaid, MLTC, and UAS requirements.
Travel locally to conduct in-person member assessments.
Field Coverage & TerritoryCandidates must be willing to work in Brooklyn, Queens, Manhattan, or the Bronx
.
Nurses will generally be scheduled for up to 3 field cases within one borough during a workday.
Nurses will not typically be required to travel across all three boroughs in the same day
.
Assignments are generally scheduled based on geographic location to minimize travel time.
Occasionally, nurses may be assigned to another borough when additional coverage is needed.
Candidates must be comfortable with local community travel and working independently in members' homes.
Required QualificationsActive Registered Nurse (RN) license in the applicable state.
Associate Degree in Nursing (ADN) or equivalent nursing education.
Minimum of 2 years of clinical nursing experience
.
UAS assessment experience is required.
Prior field/community-based nursing experience is required.
Strong clinical assessment and documentation skills.
Ability to independently assess clinical and functional needs.
Strong understanding of ADLs, IADLs, care planning, and patient safety.
Tech-savvy with strong proficiency using electronic medical records, assessment platforms, tablets/computers, and Microsoft Office applications.
Ability to complete electronic documentation accurately and efficiently.
Valid driver's license and reliable transportation for community-based assessments.
Willingness to travel throughout assigned NYC boroughs.
Fluency in Urdu, Spanish, Bengali, or Chinese required.
Preferred QualificationsManaged Long-Term Care (MLTC) experience.
Experience working with medically complex or chronically ill populations.
BSN.
Experience with other standardized clinical assessments.
ScheduleMonday–Friday | 35 hours per week
Flexible scheduling between 8:00 AM and 8:00 PM
