We are seeking a compassionate and dedicated Cardiology Nurse Practitioner (NP) to provide on-site cardiovascular care to residents of skilled nursing and long-term care facilities. The Cardiology NP will collaborate closely with cardiologists and interdisciplinary care teams.

Responsibilities include:

• Provide on-site cardiology consultations including initial and follow-up evaluations.
• Assess and manage cardiovascular conditions such as hypertension, CAD, heart failure, arrhythmias, and valvular disease.
• Order, review, and interpret EKGs, echocardiograms, labs, and diagnostic studies.
• Collaborate with cardiologists and interdisciplinary teams to develop treatment plans.
• Manage and titrate cardiac medications per evidence-based guidelines.
• Educate patients and caregivers on disease management and lifestyle modifications.
• Ensure accurate EMR documentation and regulatory compliance.
• Participate in quality improvement initiatives to reduce hospitalizations.

• Minimum 6 months experience in SNF/LTC or hospital/outpatient cardiology setting.
• Active and unrestricted Nurse Practitioner license.
• DEA license Recommended.
• BLS/CPR certification.
• Master’s degree in Nursing from an accredited NP program.
• Preferred cardiology experience (training available).
• Strong communication, organizational, and time management skills.
