What You’ll Do

• Deliver high-quality home health nursing visits across varied patient needs.

• Handle cases such as Start of Care, Resumption of Care, Wound Care, Pediatrics, and Recertifications (higher pay for complex visits).

• Collaborate with physicians and care teams for seamless continuity of care.

• Document visits via tablet within 24–48 hours.

• Stay compliant with DOH regulations while delivering compassionate, culturally sensitive care.

What You’ll Need

• Active New York RN License

• Reliable transportation + valid driver’s license

• Experience with OASIS assessments (
  preferred
  )

• Home care or acute care experience (
  preferred
  )

• Knowledge of Homecare Homebase (HCHB) is a plus
