Position SummaryFairview Skilled Nursing Home is seeking a compassionate and organized Care Plan Nurse (RN) to coordinate individualized resident care plans, collaborate with the interdisciplinary team, and ensure high-quality, resident-centered care.
ResponsibilitiesDevelop, update, and maintain individualized care plans.
Coordinate interdisciplinary care plan meetings.
Collaborate with nursing, therapy, dietary, social work, and physicians.
Ensure care plans reflect residents' needs, goals, and regulatory requirements.
Monitor resident progress and revise care plans as needed.
Maintain compliance with CMS and New York State regulations.
Current New York State RN license.
Long-term care or skilled nursing experience preferred.
Strong communication, organizational, and documentation skills.
Ability to work effectively with an interdisciplinary team.
