Work Schedule:

Day 3x12-Hour (06:45 - 19:15)

Responsibilities for this position include, but are not limited to:

• Administering medications and treatments accurately and safely in accordance with physician orders and facility policies.
• Performing medication passes and monitoring residents for effectiveness, side effects, and adverse reactions.
• Assessing, monitoring, and documenting changes in residents' physical, mental, and emotional conditions.
• Providing skilled nursing care to residents, including those with Alzheimer's disease and dementia.
• Maintaining timely, accurate, and comprehensive clinical documentation and records.
• Communicating resident status updates and care needs with physicians, interdisciplinary team members, residents, and family members as appropriate.
• Supervising, delegating, and supporting Certified Nursing Assistants (CNAs) and other assigned staff.
• Responding promptly to resident needs, emergencies, and changes in condition.
• Ensuring compliance with facility policies, state regulations, and infection control standards.
• Collaborating with the healthcare team to develop and implement individualized care plans.
• Promoting resident safety, dignity, comfort, and quality of life.
• Maintaining professional conduct, dependability, and consistent attendance while supporting a positive team environment.

To be considered for this position, you should have:

• A minimum of one year of Long-Term Care (LTC) experience required; candidates with recent experience in rehabilitation, medical-surgical, or acute care hospital settings may also be considered.
• Current BLS (Basic Life Support) certification.
• Experience administering medications and performing medication passes in a clinical setting.
• Demonstrated ability to assess, monitor, and document resident conditions accurately.
• Strong knowledge of nursing practices, resident safety standards, and infection control procedures.
• Excellent communication, interpersonal, and organizational skills.
• Experience working with geriatric residents, including individuals with Alzheimer's disease and dementia, preferred.
• Ability to supervise and collaborate effectively with CNAs and interdisciplinary healthcare teams.
• A professional, dependable, and positive attitude with a strong commitment to attendance and punctuality.
• The ability to work efficiently in a fast-paced environment and adapt to changing resident care needs.
• Successful completion of the facility's medication administration (Med Pass) test and clinical observation requirements.
