$2040.00/week -
Job Summary:
Provide licensed practical nursing care in a long-term care or skilled nursing facility setting. Ensure the delivery of quality patient care while adhering to facility policies and state regulations.
Responsibilities:
- Administer medications and treatments as prescribed by physicians.
- Document patient status, care provided, and any changes accurately in the electronic medical record.
- Assist with patient assessments, ensure safety, and support activities of daily living.
- Cover CNA duties as needed, including vital signs, hygiene, and mobility assistance.
- Collaborate with interdisciplinary team to develop and implement patient care plans.
- Participate in bi-weekly orientations and ensure all employment and work history information is complete.
Qualifications:
- Hold a valid LVN or LPN license in the state of employment.
- Possess a minimum of 1 year of experience in a LTC or SNF environment.
- Maintain current AHA BLS certification.
- Have at least one year of experience covering CNA duties.
- Provide proof of NY license or application with applied date; licensing takes 6-8 weeks.
- Demonstrate reliability and ability to work every other weekend and holidays as required.
