LPN Licensed Practical Nurse Responsibilities:

• Provide high-quality, individualized care to patients in long-term care, rehabilitation, and skilled nursing facilities

• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans, ensuring seamless transitions and optimal patient outcomes

• Conduct thorough patient assessments, monitor and report changes, and communicate effectively with patients, families, and healthcare professionals

• Administer medications, treatments, and procedures as prescribed by physicians and within the scope of practice

• Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices

• Foster a positive, supportive environment by promoting patient dignity, respect, and comfort

LPN Licensed Practical Nurse Requirements:

• Current LPN license in the State of New York

• Strong communication and interpersonal skills
