Admission Nurse

Job Summary

Admission Nurse responsible for initiating and resuming high‑quality home health services for patients referred to the agency in Queens, NY and occasionally Brooklyn.

• Create the initial comprehensive and interdisciplinary patient centered home health plan of care; update the plan in collaboration with physician or authorized practitioner (AP).
• Ensure agency compliance with regulatory and organizational requirements.
• Demonstrate expert knowledge unique to the practice of community health nursing.
• Provide high quality clinical care and teaching.

• Diploma in Nursing required;
  Baccalaureate Degree highly preferred.
• Current RN License in NY in good standing.
• Minimum one year of clinical experience in a Certified Home Health Agency.
• Knowledge of Microsoft Office and experience documenting in a clinical software system preferred.
• Driver’s License required in all boroughs except Manhattan.
• Strong assessment skills, clinical judgment, and critical thinking abilities.
• Ability to work independently in a community setting.
