Home Care Registered Nurse - Queens

About This Role

The Home Care Registered Nurse provides comprehensive nursing care for patients in their homes, utilizing the nursing process to assess, plan, implement, and evaluate patient care. This role is ideal for nurses who excel in collaboration and patient-centered care.

• Location: Manhasset, NY
• Schedule: Full-Time, Day shift

• Conduct patient interviews and explain policies and procedures.
• Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
• Communicate and document patient’s physiologic health status and plan of care.
• Assess patient’s psychosocial health status and determine coping mechanisms.
• Develop criteria for measurement of patient outcomes and establish individualized patient goals.
• Coordinate the cost-effective use of supplies, equipment, and medication.
• Implement the plan of care and maintain patient safety precautions.
• Administer prescribed treatments and provide emotional support to patients.
• Keep accurate documentation of patient’s treatment and responses.
• Participate in planning for discharge and coordinate referrals.

Education:

• Graduate from an accredited School of Nursing required
• Bachelor’s Degree in Nursing preferred

Experience:

• Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date

Licenses &

Certifications:

• Current License to practice as a Registered Professional Nurse in New York State required
• Valid Driver's License in New York State may be required depending on area of assignment
